New Patient Registration

register_now_blue_signTo register as a patient you will need to complete a registration form and provide two forms of identification.

Individuals wishing to register with the practice temporarily, whilst away from their permanent UK address will also need to provide full details of their usual doctor and surgery. We may need to contact your usual doctor as part of any treatment we provide you with and will need to forward them details afterwards, to ensure your medical records remain up-to-date.

When registering please provide us with a daytime contact number in addition to your home telephone number as there may be times we need to make contact with you promptly during opening hours. It is a requirement that any changes in personal details such as name, address, contact telephone numbers etc are communicated to the practice promptly.

Named GP

All practices are required to provide their patients with a named GP who will have overall responsibility for the care and support that the surgery provides for them.

Upon registering with the practice you will be allocated an accountable GP.  New & existing patients can obtain the name of their accountable GP by contacting the practice.

Having a named GP does not prevent you from seeing any other GP in the practice. At times your named GP may not always be available, so you may need to see a different doctor for same day advice or urgent appointments.

Practice Area

A map of our practice area and boundaries is available for viewing at reception in Cinderford.  The practice covers some 81 square miles of the northern part of the Forest of Dean, lying between the River Severn and the River Wye. The area is one of outstanding natural beauty and offers facilities for many outdoor activities. Cinderford is within easy commuting distance of Gloucester and Cheltenham. The Centre of Bristol is less than one-hour's drive away via the Severn Bridge. Newport, Cardiff and Birmingham are within easy reach.

If you move to an area outside the practice boundary you will need to register with a practice more local to your new address. Travelling distance and time for medical staff to provide home care visits for patients too ill to attend surgery must be kept to a minimum, to ensure as prompt a response as possible in meeting these needs.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
